Waiver
WAIVER AND RELEASE OF LIABILITY FORM I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH THIS EVENT, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault. I certify that I am able, have sufficiently prepared, and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in Full Life Ahead Family Weekend activities. I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity in which I may participate, and that it will govern my actions and responsibilities at said activity. I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows: (A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this activity, THE FOLLOWING ENTITIES OR PERSONS: The Full Life Ahead Foundation (FLAF) and/or their directors, officers, employees, volunteers, representatives, and agents, and the activity holders, sponsors, and volunteers; (B) INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of release or otherwise. I acknowledge that FLAF and their directors, officers, volunteers, representatives, and agents are NOT responsible for the errors, omissions, acts, or failures to act of any party or entity conducting a specific activity on their behalf. I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during The Full Life Ahead Family Weekend. I understand while participating in this activity, I may be photographed. I agree to allow my photo, video, or film likeness to be used for any legitimate purpose by the activity holders, producers, sponsors, organizers, and assigns. I agree and understand that Children's Harbor does not allow ANY alcohol or illegal drugs on campus, and that The Full Life Ahead Foundation does not allow smoking on campus during their camps. I agree that we will not use any of these things while on campus. I and all members of my party agree and understand that no pets are allowed on the Children's Harbor campus and that unless I or any member of my party have a certified service animal, we agree not to bring any pet or animal. I and all members of my party agree understand that for Safety and Security, no bicycles, tricycles, big wheels, skates, skateboards, razors, etc. are allowed on the Children's Harbor campus and agree that we will not bring any of these things to the campus I and all members of my party agree that we will participate fully in the programs designed for us. The Accident Waiver and Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.
Registration, Eligibility & Cancellation Policies
REGISTRATION: Acceptance to each Family Weekend is not automatic, and you may be added to a waiting list due to volume of registrations and needs. Leadership will make final decisions on Family Weekend attendance based on urgency of need, previous attendance, meeting essential eligibility requirements and balance of disAbilities of current registrants. You will be contacted via email to confirm acceptance of registration two weeks prior to that Family Weekend. If you are not accepted, your registration payment may be fully refunded, donated, or put toward a future camp session. ELIGIBILITY: The Full Life Ahead Foundation seeks to serve all qualified individuals with disAbilities and their families who meet the essential eligibility requirements for our Family Weekend Workshop Retreats enumerated below. These criteria are necessary to ensure not only the safety of participating campers but also their ability to receive the maximum educational benefits of the particular camp for which they have applied. Camps are organized around a family setting and a generalized age group setting. The Full Life Ahead Foundation seeks to serve eligible campers in the most integrated setting possible. All acceptances of applications are conditional. Specifically, The Full Life Ahead Foundation reserves the right to accept or deny applications or defer admission on site or prior to attendance should it later become aware that the initial application was flawed or any camper’s health has severely declined or upon demonstration that any camper does not meet the applicable eligibility criteria. All deferrals or revocation of admission must be approved by the Camp Director and the Executive Director. AN ELIGIBLE APPLICANT MUST MEET THE FOLLOWING CRITERIA: 1. Be of appropriate age. This is a family camp. The age requirement is that the individual with a disAbility must be 13 years of age or older. Siblings must be aged 5 and older; 2. The family must have a child who has a physical or mental disAbility who is 13 years old or older; 3. All family members should attend the Family Weekend to which they have applied. Please contact the Executive Director at 205-439-6534 if this is not possible and in some circumstances exception to this requirement may be made. 4. All registered family members must attend all camp sessions throughout the weekend. If an emergency arises and one or more family members must leave early, you must first speak with the Executive or Camp Director for approval. If a family leaves prematurely without approval they will be banned from future family weekends. 5. All family members must not be abusive towards themselves or others, i.e. does not physically, verbally, or sexually abuse self or others, which abuse may include hitting, biting, pushing, scratching, spitting, kicking, excessive swearing, excessive or inappropriate yelling or verbal degradation, inappropriate touching or fondling or other inappropriate behavior; 6. All family members must not pose a direct threat to themselves or others that cannot be eliminated or reduced below the level of a direct threat with or without reasonable accommodation. Direct threat is defined as a substantial risk of harm to the camper or others; direct threat may include having a highly contagious condition such as tuberculosis, hepatitis, an open or draining wound or rash, topical parasites, etc. or other conditions that may be spread through casual human contact; 7. All family members must be in acceptable health as indicated by not having a temperature greater than 98.6 orally. CANCELLATION: It is very important to the Full Life Ahead Foundation that every family who wants and needs to attend a Family Weekend be given every opportunity to do so. There is nothing worse than having an empty cabin at camp... When families cancel it creates the possibility of having an empty cabin that represents a family who could have come to experience camp and all of the knowledge, connections and HOPE that comes with it. Because of this, the following fees will apply to cancellations: - Cancellations made 14 days or more in advance of the event date will incur a 30% fee. - Cancellations made within 3 - 13 days will incur a 50% fee. - Cancellations made within 48 hours to the event will incur a 100% fee.
